New Patients

Huthwaite surgery is open to new patients and we look forward to welcoming you to our practice and to looking after your healthcare needs. If you wish to visit the surgery before registering, please call the surgery and ask to speak to the practice manager, Liz Griffin.

How to Register with the Practice

If you have recently moved to the area and are looking for a new practice, or would simply like to change practices, then Huthwaite Surgery looks forward to welcoming you.

The process is as follows:
  1. Complete a Registration Form (GMS1) and a New Patient Questionnaire. These can be obtained from reception or downloaded from the links below:

  2. Bring completed forms and 2 forms of ID (one confirming your name and address, and one with a photograph) to reception.  ID that will be accepted as proof of identity are:
    a) Passport / Driving license
    b) Bank Statement
    c) Utility Bill

 

FAQs

Can I register if I am a Foreign National?
Yes – providing you have an appropriate visa and are eligible for free NHS healthcare. We will ask to see your passport and visa.

Can I see a GP straightaway?
Yes – as soon as you have registered. We do not insist on waiting for your notes. However, it would help the GPs if you could bring a list of your medications and if possible, past medical history.

How long will my notes take to come from my old surgery?
As soon as you have registered, we will request your previous notes. These have to be sent by your previous GP, to the Health Authority who then send them to us. This can take up to 8 weeks. If deemed urgent, then this process can sometimes be fast tracked.
